The picture kind of says it all.  Are people really that smug and inconsiderate?  Well, it would look like it.  Perhaps we would let the driver off the hook if they suffered from a lactose intolerance, just finished an extra large milkshake that their dying grandmother gave them but… we don’t think that’s the case!
